How to read your CBC blood test results

The complete blood count is the test most people see first, and the one most often misread. It reports a dozen or more values with no explanation, and a single letter H or L next to a number carries very different weight depending on which line it is on.

This guide walks through every marker on a standard CBC with differential: what it measures, what a high value usually means, what a low value usually means, and when the number is worth a conversation with your provider. It is educational, not medical advice.

Who gets this panel and why

A CBC is ordered at almost every annual physical, before surgery, during pregnancy, and any time symptoms suggest infection, anemia, or a bleeding problem. It is also used to monitor people on medications that can affect blood counts.

Because it is run so often, the CBC is usually the marker set with the most history attached to it. Comparing today's values to your own past results tells you far more than comparing them to a population range.

Hemoglobin

The oxygen-carrying protein inside red blood cells. It is the single best number for spotting anemia.

If it is high
Often dehydration, smoking, sleep apnea, or living at altitude. Persistently high values are investigated for overproduction of red cells.
If it is low
Anemia. Iron deficiency is the most common cause, followed by B12 or folate deficiency, blood loss, and chronic disease. Symptoms include fatigue, breathlessness, and cold hands.

Hematocrit

The share of your blood volume made up of red cells. It tracks closely with hemoglobin.

If it is high
Usually reflects dehydration or the same causes that raise hemoglobin.
If it is low
Moves down with hemoglobin in anemia, or with overhydration and recent blood loss.

Red blood cell count (RBC)

The total number of red blood cells per volume of blood.

If it is high
Smoking, sleep apnea, altitude, and dehydration are the usual explanations.
If it is low
Contributes to anemia symptoms and is read together with hemoglobin, hematocrit, and MCV.

MCV (mean corpuscular volume)

The average size of your red blood cells. It is the fastest clue to which type of anemia you have.

If it is high
Large cells point toward B12 or folate deficiency, thyroid dysfunction, liver disease, or alcohol use.
If it is low
Small cells point toward iron deficiency or, less commonly, an inherited condition such as thalassemia trait.

MCH and MCHC

How much hemoglobin the average red cell carries, in absolute terms and by concentration.

If it is high
Usually moves with a high MCV and points to the same causes.
If it is low
Typically accompanies iron deficiency, where cells are both small and pale.

RDW (red cell distribution width)

How much variation there is in red cell size. A high RDW means the cells are unevenly sized.

If it is high
Often the earliest sign of developing iron, B12, or folate deficiency, sometimes before hemoglobin drops.
If it is low
A low RDW is not clinically meaningful on its own.

White blood cell count (WBC)

Your total immune cell count, the frontline response to infection and inflammation.

If it is high
Infection, inflammation, physical or emotional stress, smoking, and steroid medications.
If it is low
Recent viral illness, certain medications, autoimmune conditions, and nutrient deficiencies. Some healthy people simply run low.

Neutrophils

The white cells that respond first to bacterial infection and tissue injury.

If it is high
Bacterial infection, inflammation, stress, or steroid use.
If it is low
Viral illness, some medications, or, when very low, a reason to contact your provider promptly because infection risk rises.

Lymphocytes

The white cells that handle viral defense and long-term immune memory.

If it is high
Common with viral infections and usually resolves as you recover.
If it is low
Can follow acute illness, steroid use, or physical stress.

Monocytes, eosinophils, basophils

Smaller white cell subsets involved in cleanup, allergic response, and parasite defense.

If it is high
High eosinophils commonly point to allergies, asthma, or a drug reaction. High monocytes can follow chronic inflammation or a recovering infection.
If it is low
Low values in these subsets rarely carry meaning on their own.

Platelets

Cell fragments that let blood clot after injury.

If it is high
Often reactive, following infection, inflammation, iron deficiency, or recent surgery.
If it is low
Raises bruising and bleeding risk. Causes include viral illness, medications, alcohol, liver disease, and immune conditions.

How to read your CBC as a whole

Read the CBC as three related stories rather than a list. The red cell line, hemoglobin, hematocrit, RBC, MCV, and RDW, answers whether you are anemic and what kind. The white cell line, WBC plus the differential, answers whether your immune system is responding to something. Platelets stand mostly on their own.

A single flagged value inside an otherwise clean CBC is usually rechecked rather than treated. A pattern, such as low hemoglobin with a low MCV and a high RDW, is much more informative than any one flag, and it is exactly the kind of grouping Nodava does automatically when you upload a report.

Why is my white blood cell count high?
The most common reasons are infection, inflammation, physical stress, recent strenuous exercise, smoking, and steroid medications. A mildly elevated count on a single draw is very common and is usually rechecked rather than acted on immediately.
What does a low hemoglobin mean?
Low hemoglobin is anemia. The most frequent cause is iron deficiency, often from menstrual or gastrointestinal blood loss or low dietary intake, but B12 deficiency, chronic illness, and kidney disease also cause it. Ferritin and MCV usually get checked next to identify which type it is.
Do I need to fast for a CBC?
No. A CBC does not require fasting. If it is drawn alongside a metabolic or lipid panel, the fasting instructions come from those tests, not the CBC.
Can dehydration affect CBC results?
Yes. Being dehydrated concentrates the blood, which can push hemoglobin, hematocrit, and red blood cell count up without any change in the actual number of cells your body has. This is a common reason a single high reading returns to normal on a repeat draw.
